Aim: To evaluate the effectiveness of a thermo mechanical stimulation device (Buzzy®) in relation to pain, fear and anxiety during local anesthesia in children. Materials and methods: Study carried out from May 2018 to July 2019, with children aged 7 to 11 years, without previous experience involving anesthesia in the last 2 years and who needed dental treatment (extraction, restoration or endodontic) under local anesthesia in deciduous molars. The sample was randomized into a control group, which received conventional anesthesia, and an intervention group, which received anesthesia with Buzzy®. The levels of anxiety, fear and pain perception of both groups were verified using: Come Modified Picture Test (VPTM); heart rate; Behavioral Scale Come; Faces Pain Scale Revised (FPS-R) and Face, Legs, Activity, Cry, Consolability (FLACC). Results: Most children (55%) had low anxiety before and after treatment (P<0.05). The acceptability of the children to Buzzy® was 100% and the majority (90%) would like to use it again. Discussion: The tested device is an interesting tool to complement management techniques during consultations, in view of the excellent acceptability and interest on the part of patients and family members. Conclusion: This study demonstrated that the use of thermo mechanical stimulation is feasible in the dental clinic, due to its easy use and good acceptability in the clinical environment, in addition to not presenting risks in its use.

RESUMO

Aim: To verify the validity of maternal reports on the number of deciduous teeth erupted in their children. Methods: This cross-sectional study was performed with children enrolled in a Birth Cohort at the age of 12 months in the first stage and 24 months in the second stage. At both stages, children were clinically examined, and mothers reported the number of teeth of their children. Comparison between groups was performed using the Mann-Whitney non-paired Wilcoxon test. Level of agreement between two methods were estimated by the Observed Agreement, Weighted Kappa and Intraclass Correlation coefficients. Results: A total of 125 children were examined in the first stage, with mean number of reported teeth of 6.2. In the second stage, 149 children were examined, with mean number of reported teeth of 15.9. High level of agreement, kappa values and intraclass correlation coefficients were observed for both arches in both periods (p<0.001). Conclusions: Maternal report on the number of teeth erupted in children was reliable and valid. Thus, it seems to be a useful instrument for collecting data in population-based epidemiological studies targeting young children

RESUMO

The purpose of this study was to investigate the association between maternal factors and dental visits of children within their first year of life. This study used data from the 2015 Pelotas Birth Cohort and included pregnant women and their children at 12 months of age, followed-up between May 2014 and December 2015. Socioeconomic and demographic data, information on the use of dental services during the last year, and information on dental fear were collected by interviewing the mothers. Information regarding dental caries and periodontal disease were collected during the mother's clinical examination. The date of each child's first visit to the dentist was recorded. Crude and adjusted Poisson regression models with robust variance were used. The sample comprised 2,287 mother-child dyads, of which 10.1% of children visited the dentist during the first year of life. Children whose mothers visited the dentist within the last year for preventive reasons (RR: 1.51; 95%CI: 1.12-2.03) or curative reasons (RR: 1.47; 95%CI: 1.08-2.01) were more likely to have visited the dentist during the first year of life compared to those whose mothers had not used dental services during this period. Maternal tooth loss was inversely associated with a child's visit to the dentist during the first year of life (RR: 0.69; 95%CI: 0.54-0.89). Considering the mother's history of using dental services, as well as their dental caries history, is necessary to understand and improve patterns of dental health care utilization and implement public oral health policies for young children.

RESUMO

AIM: To investigate the prevalence of non-carious cervical lesions (NCCLs) and their association with individual and tooth-related factors in adults from the 1982 Pelotas Birth Cohort. MATERIALS AND METHODS: Participants were interviewed and clinically examined at the age of 31. NCCL was defined as loss of dental tissue without bacterial involvement in the cervical region. Independent variables were the socioeconomic, demographic, behavioural, and clinical characteristics. Associations were tested using a multilevel Poisson regression model. RESULTS: Five-hundred and thirty-nine participants were clinically examined, with a 26% prevalence of NCCLs, which were more prevalent in the maxilla (56.5%) and in premolars (72.9%). In the multilevel analysis, women presented lower prevalence than men [prevalence ratio (PR) 0.59 (0.48-0.73)]; those who reported smoking at both 22 and 30 years of age had more NCCLs than those who never smoked [PR 1.65 (1.31-2.07)]; and high-frequency brushers presented higher prevalence than low-frequency brushers [PR 1.26 (1.03-1.55)]. Gingival recession increased 10 times the prevalence of NCCLs [PR 10.03 (8.15-12.35)], while the presence of periodontal pockets (≥4 mm) reduced the prevalence of NCCLs [PR 0.43 (0.28-0.66)]. CONCLUSIONS: NCCLs were more prevalent in males, smokers, and those with higher frequency of toothbrushing. While the presence of periodontal pockets was associated with a lower prevalence of NCCLs, gingival recession was a strong clinical indicator for the presence of NCCLs.

RESUMO

ABSTRACT The aim of this paper is to provide a scientific basis for conducting safe root canal treatment in pregnant women, illustrating with case reports treated in the extension project Maternal-Child Dental Care at the Federal University of Pelotas. The scientific bases address the main doubts of professionals when performing prenatal dental care: use of radiographic examination, local anesthetics, prescription of medications, among other essential factors in the care of pregnant women. The assisted pregnant women sought care with dental pain. In two cases symptomatic irreversible pulpitis was diagnosed, while a third was experiencing acute apical periodontitis. Endodontic procedures were performed under controlled and standardized conditions. The root canals were prepared in a single session, using mechanical instrumentation with reciprocating kinematics (Wave One® Gold Primary - Dentsply Sirona), and the root canal length was determined by an electronic apex locator FinePex (Schuster). Root canal filling was performed by the single cone technique with Wave One® gutta-percha points. After endodontic treatment, coronal sealing was performed with composite resin A2 Z350 XT (3M ESPE). The care related to the gestational condition is highlighted, and the patients remain under monitoring. It is concluded that the use of apical locators and mechanical instrumentation, using reciprocating files associated with the single cone obturation technique with a standardized cone for each system, represents advances for the accomplishment of endodontic treatment in pregnant patients due to the shorter clinical time and greater comfort of the procedure.

Objective: The aim of this study was to answer which is the best technique to removing glass fiber post and cast metal post considering time, preservation of tooth structure, and costs. Methods: An in vitro, randomized, blinded study was conducted. Sixty endodontically treated teeth were randomized into two groups according to post type. A second randomization was performed for each type of removal technique (ultrasound, drill, or combined; n=10). One-way ANOVA was used to compare the time to post removal, paired t-test compared the amount of tooth structure removed and Kruskal Wallis followed by post hoc Mann-Whitney test were used for cost analysis (P=0.05). Results: There was no difference in post removal time considering post type or technique (P=0.630). The average loss of tooth structure in the cervical region was greater than 30% when only a diamond bur was used to remove fiber glass post (P<0.00001) and cast metal post (P=0.008). Conclusion: According to the results of this study, we can conclude that the selection of the removal technique will depend on the skill of the operator, and some loss of tooth structure will always occur, although greater loss is expected when only diamond burs are used for this purpose.(AU)

RESUMO

AIMS: To assess the presence of dental pain in child abuse victims and to compare the findings to children in the same age group with no history of abuse in a Southern Brazilian city. METHODS: This cross-sectional comparative study was carried out in a convenience sample of 68 child abuse victims from a center for psychologic support and in a comparative group of 204 students from public and private schools, all aged between 8 and 12 years. Data for socioeconomic and demographic variables were collected, and information regarding dental pain in the last 6 months was obtained via interviews with the children. World Health Organization criteria were used to assess the presence of decayed, missing, or filled teeth in primary and permanent dentition. Dental trauma was measured using the O'Brien Index, and gingival bleeding was assessed. Crude and adjusted logistic regression analyses were performed to assess the association between dental pain and history of abuse. RESULTS: Reporting of dental pain was higher among child abuse victims (54.41%) than in the comparative group (36.14%) (P < .01). After adjustment for clinical variables in the logistic regression analyses, the child abuse victims group had an odds ratio of 2.03 (95% confidence interval 1.13 to 3.64) for dental pain (P = .01). CONCLUSION: Child abuse victims presented a higher prevalence of dental pain than children with no history of maltreatment, regardless of oral health status.

RESUMO

PURPOSE: To assess the association between long-term breastfeeding and dental caries in children during the third year of life. STUDY DESIGN: This retrospective longitudinal study consisted of children who were followed-up by the Maternal and Child Public Oral Health Program. Information regarding socio-economic class, demographic status, daily frequency of sucrose intake, breastfeeding duration and oral hygiene were collected with the aid of a questionnaire. Information on dental caries in the children was collected from dental records. Negative binomial regression models were used to assess the association between breastfeeding duration and dental caries. RESULTS: We included a total of 325 children in the study. The incidence of dental caries was found to be 12.92%. Even after adjustments, children who were breastfed for a period ≥24 months were more likely to have dental caries, when compared with children who were not breastfed or were breastfed for less than 6 months. Children who had a higher frequency of sucrose intake and those with dental plaque were more likely to have dental caries. CONCLUSION: In this study, a greater incidence of dental caries was found in children who were breastfed for a period ≥ 24 months..

RESUMO

BACKGROUND: Although a high prevalence of head and neck trauma has been reported in maltreated children and adolescents, most of these studies focused on record-based analyses, and only a few used control groups. AIM: To assess the presence of dental trauma in maltreated children in comparison with children without a history of maltreatment. DESIGN: A cross-sectional study with 68 maltreated children and 204 age, sex-, and school-matched control participants was conducted in Pelotas, a Southern Brazilian city. Socioeconomic and demographic data were collected. Clinical examinations were performed to evaluate dental trauma (O'Brien Index) and dental caries (WHO criteria). Fisher's exact and chi-squared tests were used for intergroup comparisons of socioeconomic and clinical variables. Multivariate logistic regression analyses were conducted to obtain adjusted odds ratios (OR). RESULTS: The prevalence of dental trauma was 23.53% (n = 16) among maltreated children and 14.22% (n = 29) in the comparative group. After adjustment, the occurrence of dental trauma showed association with child maltreatment, with an OR of 2.14 (95% confidence interval 1.03-4.44) for maltreated children in comparison with children in the comparative group (P = 0.04). CONCLUSIONS: Maltreated children presented with more dental trauma than those with no history of maltreatment.

RESUMO

This study investigated the effect of human milk, alone and associated with sucrose, in the cariogenicity of biofilms in a microcosm biofilm model and compared with the cariogenicity of sucrose and bovine milk. Microcosm biofilms were grown in enamel discs in 24-well plates. Six growth conditions were studied: DMM (chemically defined artificial saliva - negative control), DMM with 1% of sucrose (DMM+s) (positive control), human milk with DMM, human milk with DMM+s, bovine milk with DMM, and bovine milk with DMM+s. After 5 days, the outcome variables surface hardness change (%SHC), microbiological composition of biofilms, and pH of supernatant were analyzed. All groups had significantly lower hardness loss compared to the DMM group with 1% of sucrose. Human and bovine milk associated with sucrose showed higher hardness loss. The supernatant pH values after 6 hours of different treatments were similar for the groups sucrose and human milk associated with sucrose (p>0.05). After 18 hours at rest in pure DMM, an increase in the pH of the supernatant was observed. Higher values of total microorganisms count were found for sucrose and bovine milk groups compared to the group supplemented only by DMM. Bovine milk group showed greater amount of total aciduric microorganisms in comparison to human milk group. Within the limits of this study, it can be infered that both human and cow milks have some cariogenic potential, although differing from sucrose in terms of mineral loss.

(AU)

Objective: to assess whether maternal eating habits reported during pregnancy influence their children's sucrose intake in the first two years of life. Subjects and method: a total of 204 mother-child dyads followed-up by a maternal and child oral care program in southern Brazil participated in the study. The independent variables were obtained from dental records provided by the program. The maternal eating habits were collected during pregnancy and the daily sucrose intake of the children was based on a frequency food questionnaire dichotomized in <4 times/day and ≥4 times/day. The multivariate analysis was performed using Poisson regression models with robust variance to estimate the prevalence ratio and the 95% confidence intervals. Results: the prevalence of sucrose intake in the first two years of life was high. A high sucrose intake (≥4 times/ day) was detected in 90.69% of the children. After adjustment, the outcome (high sucrose intake) was 10% higher for children whose mothers reported drinking sugar-sweetened beverages to satisfy their thirst during pregnancy [PR = 1.10 (95%CI: 1.02-1.18)]. Conclusion: most of the children presented a high sucrose intake at an early age. Screening pregnant women for sugar- -sweetened beverage intake may be a potential way to recognize the children at risk for future high sucrose intake in early childhood. (AU)

RESUMO

OBJECTIVE: To investigate the occurrence of developmental defects of enamel (DDE) in the primary teeth of children aged 24-36 months and their association with early-life events. METHODS: This study was undertaken with children aged 24-36 months in Pelotas, Southern Brazil. Information on demographic and socioeconomic characteristics, events occurring during pregnancy and neonatal events was collected previously. In this study, information on the health-related behaviours of mothers and children was collected using questionnaires and information about children's oral health was achieved through clinical oral examination. The outcome, the prevalence of DDE (≥1), was determined using the DDE index (FDI). Bivariate and multivariate logistic regression analyses were performed. RESULTS: Five hundred and three mother/child dyads were evaluated, and 13.1% of the children had DDE. Multivariate analysis showed that children with Apgar scores less than 7 had around 2.5-fold higher odds of developing DDE than children with Apgar scores between 7 and 10 (OR = 2.78; 95% confidence interval [CI], 1.35-5.73). The analysis of different types of enamel defects shows that the presence of hypoplasia (OR 3.13; 95% CI, 1.35-7.27) and diffuse opacities (OR 3.95; 95% CI, 1.68-9.27) remained associated in adjusted analysis with Apgar scores. CONCLUSION: Early-life events may cause DDE. A lower Apgar score was associated with a higher prevalence of enamel defects.

RESUMO

Objective: this study aimed to assess the use of dentalservices in a southern Brazilian city by children aged12-18 months and to verify the relationship betweensocioeconomic and demographic variables, as well asmaternal use of dental services, and presence of dentalcaries in the children. It also aimed to describe theprevalence of use of dental services by variables of disparity.Subjects and method: this cross-sectional studywas performed on the National Children's VaccinationDay in 12 public health care centers in Pelotas, RS, Brazil. The data on demographic and socioeconomic status(children's sex, number of children, family income, andmaternal education), pattern of maternal use of dentalservices, and children's first dental visit were collectedusing a semi-structured interview answered by themothers. Clinical examination was performed to assessdental caries in the children. Chi-square and Fisher'sexact tests were performed to analyze the associationsand an equiplot graph investigated disparities in the prevalenceof the first dental visit of preschoolers. Results:most children had never been to the dentist (93.4%).The bivariate analysis showed that children who hadnever been to the dentist belonged to families with lowerincome, and their mothers presented low level ofeducation and did not use dental services. Conclusion:most children aged 12-18 months had never been to thedentist and there is a socioeconomic disparity in the useof dental services by preschool children. (AU)

(AU)

RESUMO

PURPOSE: This study's aim was to evaluate the health-related quality of life (HRQoL) and associated factors of maltreated children and adolescents. METHODS: This cross-sectional study consisted of individuals between 8 and 17 years of age who were victims of maltreatment. The KIDSCREEN-52 was used to measure their HRQoL. Socioeconomic and demographic data were collected, as well as information about the type of abuse, type of perpetrator, and psychological support adherence. The Kruskal-Wallis test was used to analyze the scores of the KIDSCREEN-52 by the independent variables. RESULTS: One hundred and thirteen victims answered the questionnaire. Girls and adolescents had a lower overall HRQoL score (p < 0.05). Children who suffered from sexual abuse had a lowest score on the moods and emotions dimension than children who suffered from physical abuse (p < 0.05). There was a lowest score on the autonomy dimension among the children who suffered maltreatment by an extrafamilial perpetrator than those who were victimized by an intrafamilial perpetrator (p < 0.05). A lower score on the psychological well-being was observed among the children who adhered to the psychological support program provided by the service compared to those who terminated the service before completion (p < 0.05). CONCLUSIONS: Adolescents and girls were associated with lower HRQoL scores. There was no association between the overall HRQoL score and the characteristics of the maltreatment; however, the type of abuse and the victim's relationship with the perpetrator were only related to some dimensions. These findings may assist maltreatment care providers in planning strategies to reduce the consequences that violence may cause.

RESUMO

This study aims to assess and compare Oral Health-Related Quality of Life (OHRQoL) in child abuse victims and non-abused children in a Southern Brazilian city. The study compared two independent samples of children aged 8-10 years: 48 child abuse victims recruited from a centre for Child and Adolescent Psychological Support (NACA) for abused children, and 144 public and private school students. Data collection consisted of administration of the Child Perceptions Questionnaire 8-10 (CPQ8-10) to measure OHRQoL (dependent variable), clinical examination (dental caries), and collection of socioeconomic and demographic information (age, sex, skin colour, family income, and type of school). Multiple linear regression models were used to assess the association between presence of abuse and OHRQoL and subscales. After adjustment for clinical and sociodemographic variables, child abuse victims were found to exhibit higher CPQ scores on the overall scale and on the oral symptoms and functional limitations subscales. In conclusion, child abuse victims have a higher impact on OHRQoL. Based on the results, it is possible to suggest that greater care should be taken of these children, not only in providing treatment for oral disorders, but also in providing interdisciplinary care.

RESUMO

Abstract This study investigated the effect of human milk, alone and associated with sucrose, in the cariogenicity of biofilms in a microcosm biofilm model and compared with the cariogenicity of sucrose and bovine milk.

RESUMO

AIM: To assess the effects of an oral health education strategy targeted at students with disabilities. METHODS: This quasi-experimental study included students aged 7 to 24 years. The intervention was conducted for 5 weeks and divided into 2 phases: (1) comprising supervised toothbrushing and educational activities to students; (2) verbal oral health instructions and oral hygiene demonstration targeted at the caregivers. A clinical examination was conducted to evaluate the dental debris and gingival bleeding before the intervention, and the Wilcoxon test was performed after phase 1 and 2 for comparisons between paired samples. RESULTS: A significant decrease in dental debris and gingival indices compared with baseline occurred after phase 1 (p < 0.05). After the meeting with caregivers (phase 2), there was significant decrease in the debris-index (p < 0.05). CONCLUSIONS: Providing a simple and short-term educational intervention to students with disabilities had a positive impact on their oral health.

RESUMO

This study aimed to investigate the influence of specific maternal-factors on caries prevalence in offspring. This cross sectional study was conducted in Pelotas, Brazil nested in a cohort of adolescent mothers with children aged 24-42 months. A questionnaire was administered to collect information in relationship to socio-economic, demographic, and behavioral maternal variables. The outcome (children's dental caries prevalence - dmfs > 0) was collected from clinical examinations. Bivariate and multivariate analyses were conducted. A total of 538 mother/child dyads were evaluated and 15.1% of the children exhibited caries. Adjusted multivariate analysis showed children from mothers living without partners (p < 0.027) had more caries than those living with partners. Maternal perception of a child's oral health was associated to occurrence of caries, particularly when mothers classified their child as poor and these children had a higher level of caries. In addition, mothers accurately evaluated their child's teeth when associated with caries occurrence. Maternal oral health care practices were associated with children's caries prevalence. Our results demonstrated mothers accurately evaluated the oral health of their offspring.

RESUMO

Objective: To identify the relation between the use of continuous medication by children with neuropsychomotor disorders and the development of the dental caries disease. Material and Methods: The data were obtained from medical and dental records of children, who were assisted in an extension project of the Dentistry School of the Federal University of Pelotas. Socio-economic variables, the disability diagnosis, the type and frequency of the continuous medication, and the dental caries experience registered in the dental record were collected. The data were evaluated by double typing, analyzed by descriptive statistics, and the associations were tested by the Qui-squared, Fisher exact, Mann-Whitney, and Kruskal-Wallis tests. The Stata Statistical Software, version 12.0, was utilized considering the level of significance of 5%. Results: 119 records were evaluated, and the results were distributed by age bracket due to the high age range. Undoubtedly, the cerebral palsy was the most prevalent (33.9%). From the total of the children, 68.8% were using continuous medication, and the majority (56%) uses medication there are more than 24 months. The anticonvulsants were the most used medications (33.8%), and 96.5% of the children used medication during the nocturnal period. Half of the utilized medications presented sucrose in its composition. Conclusion: A statistically significant association between the use of continuous medication, containing sucrose, and the dental caries experience were not observed, what suggests that other risk factors contribute to the disease installation.
